The letter outlines the mandatory documents required for processing the case for prior sanction, including a copy of ECHS Card, personal application of ECHS beneficiary, estimated cost of treatment from Non Emp Emp hosp, undertaking for acceptance of CGHS rates from ECHS beneficiary, statement of case with proper justification of advance sanction, other relevant documents, and revised proforma. The letter is signed by Lt Col Jt Dir (Med) for MD ECHS.

PRIOR SANCTION FOR TREATMENT IN NON EMP HOSP
- Please refer CO ECHS letter No B/49770/AG/ECHS dated 05 Oct 2018 and Para 18 (a) of SOP on Treatment Management in ECHS.
- The mandatory documents required for processing the case for Prior Sanction for treatment in Non Emp Hosp as per mentioned below:-
(a) Copy of ECHS Card.
(b) Personal application of ECHS beneficiary.
(c) Estimated cost of treatment from Non Emp Emp hosp where ESM wants to take treatment.
(d) Undertaking for acceptance of CGHS rates from ECHS beneficiary
(e) Statement of Case alongwith proper justification of advance sanction.
(f) Other relevant documents.
(g) Revised Proforma as per CO ECHS letter No B/49770/AG/ECHS dated 20 Oct 2020.
