SUB: LIST OF PRIVATE HCOs EMPANELLED IN CGHS GWALIOR UNDER ADMINISTRATIVE CONTROL OF CGHS KANPUR FOR THE QUARTER ENDING SEPTEMBER-2021 UNDER CONTINUOUS EMPANELMENT SCHEME-2017.
In compliance of the Ministry of Health & F.W, Nirman Bhawan, New Delhi O.M. No. S.11045/36/2012-CGHS (HEC) dated 26th December 2017 and subsequent CGHS Hospital empanelment Committee recommendations on the subject cited above a list of approved HCOs in CGHS, Gwalior under Administrative control of CGHS Kanpur for a period of two years or till further orders whichever is earlier (copy enclosed “Annexure-A”)
The list of approved HCOs will be effected from the date of issue of this O.M. for providing medical facilities to the CGHS beneficiaries according to the terms & conditions laid down in MOA signed by the authorized signatory of the above listed HCOs.
A copy of this office order along with a list of empanelment HCOs are placed on the website at cahs.gou.in
