Subject : Fresh Empanelment — HCGPANDA CANCER Private Hospitals Exclusively for Cancer under continuous Empanelment scheme -2017, under CGHS, Bhubaneswar reg.
In pursuance of Ministry of Health & Family Welfare, New Delhi Guidelines-2017 as available in the website of Ministry of Health & F.W, New Delhi on the above mentioned subject (for continuous empanelment scheme-2017) for empanelment of new Health Care Organization (HCO’S), it is hereby intimated that the said Health Care Organization (HCO’S) whose name is shown below, has been empanelled under CGHS, Bhubaneswar for a period of 02 (TWO) Years w.e.f 14.9.2021 or till next empanelment which ever is earlier.
The Health Care Organization (HCO) who has qualified to be empanelment under CGHS, Bhubaneswar has signed the Memorandum of Agreement with CGHS, Bhubaneswar and submitted all other requisite documents and have accepted the rates of 2014 of CGHS, Bhubaneswar uploaded in the website of CGHS, Bhubaneswar.
